You will have the ability to access a minor child’s account by completing the activation by using the child’s personal information and your cell or email associated with the child’s account. Please allow 24 hours for your images to populate your patient portal. We are glad to have you as a patient and hope you will enjoy being able to access your imaging reports and schedule an exam quickly and easily online. Screening Mammograms, general x-ray, DXA and some ultrasounds can be scheduled through the portal. All other exams such as CT and MRI should be scheduled by calling 480.610.7400 Support for any portal issues including log in assistance is available Monday to Friday from 8 a.m.

to 4:30 p.m. MDT at (480) 684-6000 or toll-free at (855) 355-6500. Portal requests are approved during standard business hours 8-5 M-F. You will have full access once Banner staff reviews your request to verify that the request is from a parent or legal guardian and validate it against the patient's medical record. Upon approval, authorization will remain in effect until the child's 12th birthday. Account access will be revoked automatically on the child's 12th birthday.
